  • I agree to comply with the rules and policies of Bridgend College and partner institutions (including plagiarism, bullying and harassment, fees, disciplinary, ICT Usage Policy and attendance policies), abide by the provisions of the Health and Safety at Work Act and to conform to the no smoking policy on all Bridgend College premises.

I agree to pay all fees as they become due, and to the release of information concerning my progress, achievement and attendance to my Sponsor, Training Provider or Employer where I am released from work to attend the course as appropriate and to my parent/guardian if I am under 18 on the first day of my learning programme.

I confirm that I remain liable for the full fee as per the College and University regulations if I withdraw early from a course where a fee is payable.

I agree that should I fail to pay all fees and charges as they fall due, I will become liable for all administration costs incurred by Bridgend College in recovering my debt.

Should my sponsor/employer not pay my fees as agreed, I will be liable to pay these fees as they become due. In signing this document I confirm my understanding that the learning I am undertaking may be partly financed by the European Social Fund through Welsh Government.

I understand that failure to disclose a criminal conviction under the Rehabilitation of Offenders Act could jeopardise my place on a course and progression opportunities at Bridgend College. Spent and unspent convictions could impact on your ability to gain employment within certain professions and will still show on a Data and Barring Service (DBS) check. For further information please visit nacro.org.uk
I accept that I participate in college enrichment and sporting activities at my own risk and that liability for injuries or damage caused as a result of such participation is at my own choice and risk.

I accept that work created as part of a course at Bridgend College, along with any outputs (photographs, news stories, achievements etc) may be used by the college for marketing purposes.

Bridgend College uses the data on this form to assess student learning needs and will send some of the information it holds about you to the Welsh Government and other approved users which may include academic researchers and commercial bodies. Such personal data is treated in confidence and will not be disclosed to any third party except where the college is required to do so by law or to plan support. This information forms your learner record held by the Welsh Government.

Information we hold about you will be sent to the Welsh Government and used in the following ways:

    • By the Welsh Government and other public bodies, to carry out the funding, planning, monitoring and inspection of learning, and to produce statistical publications.
    • By selected third parties, including social research organisations, to carry out research, analysis or equal opportunities monitoring relating to education and training.
    • By the Welsh Government and third parties acting on its behalf, to assist in linking your records to other data sources for the purpose of statistical research. • The Welsh Government may share your name, date of birth, address and postcode with electoral registration officers taking part in data matching pilot schemes to improve the accuracy of electoral registers.
    • Your contact details may be passed on to contractors acting on behalf of the Welsh Government (including the Wales European Funding Office), to undertake interviews or surveys of current and former learners, as part of formal evaluations of Welsh Government funded programmes. Contractors will contact a sample of individuals. You may be contacted and asked to take part in an evaluation of your personal experience of a learning activity.

If you are contacted, the purpose of the interview or survey will be explained to you and you will be given the option to say yes or no to taking part. Contractors will use your details only for the purpose of carrying out the evaluation and in accordance with the Data Protection Act. Your details will then be deleted once the evaluation contract is complete.

    YOUR RIGHTS:
Under the Data Protection Act 1998, you have rights of access to the data the Welsh Government and Bridgend College holds about you. You may have to pay a small fee for this. For further information please visit the Bridgend College website or wales.gov.uk/llwr or email llwr@wales.gsi.gov.uk

The data you supply will also be used by the Learning Records Service (LRS) to issue you with a Unique Learner Number (ULN) and share information about your learning with organisations linked to your education and training. For further details of how your data is shared and used by LRS and how to change who has access to the record, please see the LRS website at learningrecordsservice.org.uk

Your submitting this form constitutes consent to the above declaration.
